This is a recommendation for the creation of a page on this site that would allow people to complete a survey about a variety of factors involving their practice of the KSMO tq. This could range for marketing questions such as how they found out about the program to how often and long they've been engaged in practice, do they have a partner, what sort of echo effects there are, are having KSMO's or still with the ejaculatory orgasms, etc. In terms of structure of the form, it would seem that radio buttons should suffice, meaning that the answers would likely be multiple choice. If it is possible for a fill in the blank 'other', that would be great. I'll end for the moment and begin working on a more formal questionnaire and send it later.
